Description: An interactive session to explore key concepts in and tools for person centred decision making and palliative care communication. This sessions aims to increase the health care provider’s comfort level in approaching challenging conversations during the COVID-19 Pandemic and beyond.

• Discuss recent palliative care literature that challenges trends in practice
• Summarize practice changes supported by evidence
• Identify challenges to the application of evidence-based practice changes in rural and northern communities
