PROGRAM DESCRIPTION
An interactive session to explore key concepts in and tools for person centered decision making and palliative care communication. This session aims to increase the health care provider’s comfort level in approaching challenging conversations during the COVID-19 Pandemic and beyond.

OBJECTIVES
By the end of the session, participants will be able to:
• Illustrate various approaches to facilitating essential discussions related to palliative and EOL care
• Discuss Advance Care Planning, Goals of Care and Treatment
